Click On The Carrier Name For The Needed Forms:

BCBS:

AMG will request the Tier I claims data on your behalf on BC/BS groups we service.  If the group was not submitted thru AMG, please provide a Current Census, Company Tax ID and the completed HB2015. To renew a BC/BS case as is, submit the MSP form received by your client.  If a plan change is made, send the amendment form with the MSP. If you wish us to quote a new case with BC/BS, please complete the Gatekeeper Form.

UHC:

Complete the HB2015 and HB2015 Additional Information Form (for claims over $15,000) include a copy of the renewal census with any changes.

To renew a UHC written thorugh AMG send an email to Heather Matias Ainesworth .  To make changes, complete the renewal change form included in the renewal packet and forward to Heather.

All Other Carriers:

Complete the HB2015 and include a census along with the client Tax ID.

It can take up to 30 days to receive the claims information so act early on your renewals!
